Without a thumb, I would just hold the middle of the pencil between the base of my index and middle finger, and again use my middle finger for the up and down motion while my wrist moves side to side. The only way its different is that I moved the pencil from the outside of my index finger to the inside of the index finger. It still pivots around the same point.

This is scary
. That's exactly how I write and hold a pen today! They tried to force me to put my thumb down in the "proper" way at school, but it was just too uncomfortable. My handwriting is by "normal" girl's standards terrible and huge, but apparently legible enough to get very exceptionally high marks in exams and draw well apparently. This is why I always type up any correspondence/assignments now. This is why I love computers so much.
