one of the questions asks how old the child diagnosed with "autism" is. I almost quit the survey, but I figured they could throw out my responses if they didn't fit criteria. What if my child has another diagnosis - autistic disorder, PDD-NOS, Aspergers, NVLD, Rett's Syndrome... etc. What is the criteria for the subjects? This is not clear and should be made clearer.
Another question asks male/female... I wasn't sure if it was referring to my child or me?
This survey needs some clarification.
