Someone says it's "all in your head" if you're imagining something. If a loved one died, that is obviously external and happened outside of one's head. The way you view the world, the differences of autism, however, <i>are</i> in your head because no one else can see from your point of view.
Someone dismisses something as all in your head if they don't believe you, or think you just need to get over it which is a sign that they don't understand or think you're whining.
