My motto is to always apologize when I have made a mistake - this is not a rare occurrence; and I accept that others make errors too. As long as we are human, we will all make mistakes. I will say, "as explanation but not as an excuse..." I was tired, had been sick, got caught up in another emergency. I also tend to put this information in an email which gives the recipient a chance to cool down before responding.
