I'm filling out SSDI for work history since I first started having problems working April 24, 2014, but there's something about the form I don't get.

Under question 4, they ask for payment beyond regular pay such as sick pay and disability pay. They want you to put the type, the name of the employer, how much and how often you were paid, and the date received in a date range. This confuses me.

I received my disability payment only once so far, on July 7th, covering a period from mid-May to mid-June, but I'm not sure how to write that on the form. I also took a couple days off with vacation day, but the way they ask for the information I don't know how to write it. I also received my monthly commission from my employer, which varies each month.

How would I put it on a disability form that is formatted like this?



OK, disability payment was first day of disability 05/15/2014, with a one-time gross payment for the period from 05/22/2014 through 06/10/2014 of $1074.05, with a weekly benefit amount of $383.59.

The monthly commission that our whole team gets paid at the end of the month, it was $204.69 on 5/30, $0.00 on 6/27, and $223.44 on 7/25. I'm not sure how to add it to the form that is mentioned above.

I received $128.24 for vacation time on each my 7/18 and 8/1 checks.

I have no idea how to put this information into the box above.

They also have a stipulation: "We must see the original document(s) or a certified copy of the item(s) from the custodian of the original record(s). We cannot accept photocopies except for tax returns." The problem, though, is that my pay stubs are electronic and I organized them into PDF files so I can print them out when needed, but I don't know if these would count as the "original document(s)" or "a certified copy of the item(s) from the custodian of the original record(s).

The only pay stub I received in paper was the one for short-term disability (of which I also made an electronic copy).

This is complicated and I'm not sure I can complete this successfully on my own.

I also need to fill this out, in terms of co-pays, medicines, etc., to help with my medical treatment:



I paid a lot of this with my HSA (health savings account) card, in which I set a certain amount of pre-tax income aside for medical expenses. They said not to "show expenses that have been or will be paid by an insurance company, other organization, or other person." Does the HSA count as an "other organization" or "other person"?

Should I do an average of the expenses during the period since I started getting sick on April 24, 2014?

Do you have an attorney versed in disability and SSA Law? If so, I'd be asking him or her these questions. If not, I would not bother completing this application until you have talked, and possibly hired an attorney.

I tried talking to one, but he only handles appeals. He gave the impression that an attorney will only work with you once you've been denied and are going to appeal.

I guess, though, there are attorneys who help out with the initial process?

There are some attornies that will handle your case from application process through the appeal. Check with your local bar association or legal aid society for assistance.

how is it that you are getting payments already if you have not yet applied?

I suspect she also has private disability insurance through her former employer.

I've heard that most people get denied when applying for SSI/SSDI for the first time. I've also heard that many of these people become accepted for benefits through appeal.

Actual still current employer. I'm back on disability through them.

All attorneys tell me I must go into the Social Security Office for help with that. I guess I can block out the entire day tomorrow for waiting at the Social Security office for someone to help my benighted self.

It's not like there's anything else I'd rather do.
