Umm lets see:
Possibly Autistic (age 5-6) (was somewhat non verbal had issues with loud sounds bright lights)
ADD or what is now known as ADHD (No longer an issue)
OCD (hence the Billy Joel Knowledge) I'm unsure if it's truly OCD now or a side effect of Autism
(the following were brought on by some trauma I endured when I was in school at age 6 and are no longer an issue in my life now)
possible Schizoid disorder I disagree with this
Depression
Suicidal (age 10) Yea I tried to hang my self a teacher saw me and naturally she freaked.
Possibly Autistic (age 12) (unsure if it would have been Aspergers or Classic Autism Tho I wished my folks would have looked more into this)
--------------------------------
Some depression issues in my teen years (typical teenaged crap)
Major Depression Disorder in 2007 after a series of family deaths.
Autism Spectrum Disorder Level 1 (with or without intellectual impairments). (I paid to get an official screening in12/13)
Autistic Spectrum Disorder In Partial Remission. (according to a doctor that VR paid for 8/14, I don't think Autism is something that can be in "remission")
Specific Learning Disability In Mathematics.

Those are some of the official diagnoses I got now however if you want to count unofficial diagnoses I was told that I was "ret*d"by a teacher or two when I was in middle school. Was informally labeled as EBD Tho me and my parents disagreed with the EBD titles so that is why it wasn't formally labeled as such
Finally, I suspect heavily that I have Dyslexia and Disgraphia tho I don't have the bucks to spend to get yet another screening.

Asthma age 14

Eczema at different times of my life, the same with eye infections and ear infections, and one other infection when I was about 20, a sinew ganglion at 19 and pneumonia a few years ago.

Allergies age 14

Asperger's age 31

Edit to add: all these diagnoses are correct (or were in the cases of temporary infections)

By 'formal', I simply mean I got these diagnoses given to me in person by a doctor on a particular day. 'Informal' means I have the label, but it's not clear when I was given it and by whom.

Formal diagnoses
Crohn's disease
Autism Spectrum Disorder
Hashimoto's thyroiditis

Informal diagnoses
Generalised anxiety disorder
Depression/Depressive disorder/Recurrent Depressive disorder
Emotionally Unstable Personality Disorder (you wot, m8? :? I won't go into the backstory now, but literally only found about this one yesterday)
Atypical autism (once again this involves a long backstory, but I managed to get it changed to ASD!)

Aside from the last two, all were correct. I also think I have ADD but have never received or seeked a diagnosis for that. Have thought about it, but on the whole I don't think it's worth it.

Other than autism, I have been diagnosed with ADHD and a Learning Disability in the second grade. I can't recall any other diagnoses I've had, so those are all I've been told. ^^ I'm still questioning whether I have ADHD or not... but yeah. I was also diagnosed with having seasonal allergies. It used to be really bad when I was younger, but they've gotten much better over time.

Self-diagnoses I have include an intellectual disability, dyscalculia and generalized anxiety disorder. However, doctors and psychiatrists have told me these have to do with my autism diagnosis and aren't really needed as separate ones. I also suspect that I have a moderate to severe form of Separation anxiety. It's hard for me to be without my parents in certain situations, which can usually lead to severe meltdowns if they can't be at my side at times.

I was diagnosed with OCD at 18 and take Anafranil for it.

Diagnosed with schizo-affective/bipolar disorder and depression a few years later and put on Zyprexa and Zoloft.

ADD a couple of years later and on Adderal.

Anxiety disorder around the same time and prescribed colonzapam.

I'm still on all this stuff except the Adderal and Zyprexa. I take Serequel XR instead of Zyprexa.

I don't feel the symptoms of schizo-affective/bipolar as much as I used to, and the side effects (such as akathesia) are much worse than any mild discomfort i feel due to mood instability. I take the serequel xr as needed now (which decreases the severity of side effects).
