I'm supposed to "hang out" with a girl that I dated a few months ago but because of busy schedules, kind of drifted away from. I have no idea how to start up the dating process again after a long hiatus, its not like we don't know each other fairly well, but we will probably be somewhat awkward since we haven't spoken in so long. What in the heck should I plan for us to do?

I've been in this same situation before, and last time I screwed it up royally by keeping it too formal like a first date and basically boring the girl.

Try just getting back together as friends first, to allow time to re-adjust to any possible rebirth of the relationship...

Stand facing her, observe her pretty face, reach out and gently take her hands in yours, tell her that you have missed her and give her a hug. If she's good for it hug her for a while. Hell, women feel pretty good to hug anyway! Good luck. :D
