I have problems when I order something at a fast food or restaurant. I have trouble understanding people over the noise. And I also have other problems, like when at Panera I didn't remember whether I order a cup or a bowl of soup. And I always forget my change, and the people start dumbing down their speech and look at me like a have a rail through my brain.

I don't like it in social settings when I come across as stupid, but the situation easily overwhelms me. Yesterday I sat down to eat and found out they messed up my order, but I didn't do anything about it because I'd rather skip part of my meal than have to go back up to the counter and relive that. 8O

Just keep practising at it, or choose quieter eateries. The busier a place is, the less likely they are to have me as a patron.

You can also try going at less busy times & trying to speak up when ordering more. I don't feel that much anxiety nowadays about ordering but I sometimes have problems. I have low vision & reading the menu on the top behind the counter is a problem for me & sense I'm picky & like routines I stick with ordering the same things but what those things are change for what fast food place I'm at & my dyslexia sometimes acts up & I get what I'm ordering for that specific place or get it confused with an item for another place. Places sometimes get my order wrong too but I'm usual OK with going back for the most part; I like my hamburgers plain or plain with cheese & they don't get the fact I want it plain or plain with cheese. I ask for salt & pepper if they don't keep it out & I sometimes have to ask a couple times; like when McDonalds ask if I want any sauce with my nuggets I say No but I'd like salt & pepper but they don't key it in on the order of coarse & when they the person who took my order starts putting things on the tray they've forgotten or someone else is putting it on so I have to ask again.
