I assume you're forming in South Carolina. The process is very straightforward.

Make sure your name is not already taken. This doesn't have to be your real name, but try to be conventional (there are naming rules, they vary by state).
Remember to include "LLC" in your name (long form is OK too).

The fields are:
1. The name you've chosen and checked out.
2. Your address (your principal place of business, this can be your home, if you work at home).
3. You and your home address.
4(a). You and your home address (starting to notice a pattern?).
4(b). leave blank, unless you're forming this LLC with someone. If so, you'll need to take a few extra steps (PM me).
5. leave blank.
6. leave blank.
7. leave blank.
8. leave blank.
9. optional. If you leave this blank, you'll operate under South Carolina statute (Uniform Limited Liability Company Act of 1996), you can find it here: http://www.scstatehouse.gov/code/t33c044.php
There are no compelling reasons to have an operating agreement if you're operating solo. At least none spring to mind at the moment.
10. Your signature and date.
Follow the instructions on page 2 (provide 2 forms, send payment, etc)
Normally I would have it notarized, but that's up to you.
An important thing to remember: LLCs don't go away until you file for dissolution (in case you ever decide to discontinue your business).
Also very important: Keep meticulous accounting records, and use a separate bank account for business transactions. Don't use your business account for anything personal. Very bad things can happen if you use it for personal transactions. Also make sure to maintain a balance in your business account for basic operations (don't keep it drawn down below operating levels). The last thing you want is for a judgement against you (or the IRS) to 'pierce the corporate veil'. Hope it helps.
