It means usually that the person disregards another, doesn't really care about them and probably wouldn't ever.
In what situation did you hear this figure of speech?
It doesn't necessarily mean that the person had one thought about them and then didn't have a second thought about them. Though you could extend that literal interpretation out and get the jist of what it means. A person who says, "She wouldn't give him a second thought" usually means that she does not like or care for the guy romantically (her so-called first thought) and would not reconsider (this reconsidering and changing her first thought would be considered the so-called second thought) him and be interested in him romantically. She may have had multiple different thoughts about the guy, but this figure of speech refers to certain categories of those thoughts. Say for instance, she may have wondered where he was when he missed class but this has nothing to do with the topic of whether or not she likes him romantically, so it not part of this thought category and isn't classed as the second thought.
Second thougĥts are usually the opposite of the first thought or in some sort of opposition to first thoughts.
