I hate it when I accidentally turn on my smartphone's flashlight and it starts shining in my eye.

I've had that happen too. I don't know how it came on accidentally. Maybe my finger slipped or something. It's so annoying.

1. Disable gesture / button / whatever torch activation
2. Remove it from quick tiles and similar

That's what I can think of.
